Se consigue usando paths, es decir, el Freeform pen tool (lo siento, mi photoshop está en inglés) y las herramientas relacionadas. Normalmente hay que borrar bastantes de los puntos que genera el freeform y ajustar las curvas, pero los resultados son muy buenos.
